Responsibilities:
  • Transportation of residents to and from scheduled appointments in a timely and safe manner.
  • Assure the safety of each resident when transporting to and from the doctors office and/or hospital.
  • Accompany ach resident on all medical visits. Stay with resident(s) until visit is complete.
  • Observe the following guidelines at each visit:
  • Sign in resident upon entrance.
  • Present residents documents to office personnel.
  • Before leaving doctors office, please observe the following:
  • Obtain residents documents.
  • Obtain any new prescriptions.
  • Ensure doctor has written on the doctors order form.
  • If follow-up appointment necessary, obtain appointment cards.
  • After returning to facility, report to RCM and be prepared to answer questions concerning visit.


Qualifications:
  • Physically able to help ambulatory and non-ambulatory residents transfer to and from wheelchair to vehicle.
  • Possess a valid state drivers license.
  • Clean driving record.
  • Clean criminal record.
  • Long term care experience preferred.

  • Verbal ability is required to communicate with residents and to understand instructions received from nursing staff.

  • Ability to read and write and accurately report.

  • Must be 25 years of age or older.
  • CNA required
