We are looking for a full-time Street Outreach Coordinator to join our team at our Gallup offices. We offer a competitive salary range from $66,000 - $70,000 (BOE) and the Benefits shown below. Summary:

Under the supervision of the Operations Manager, the Street Outreach Coordinator is responsible for coordinating outreach initiatives by managing Case Managers and coordinating with consortium partners. Duties include but are not limited to, overseeing Case Managers assigned to outreach initiatives, coordinating meetings with community partners and grant funders, and compliance with local, state, and federal regulations. The Street Outreach Coordinator will work under the supervision of the Program Director and will also work closely with the administration team, medical staff, clinical staff, and contractors to ensure the overall well-being and safety of clients and staff.


The Street Outreach Coordinator is a partially grant-funded position that will coordinate activities consistent with the HRSA grant for the Four Corners Rural Health Care Services Outreach Project and other related outreach initiatives. The HRSA grant project aims to significantly improve health outcomes for individuals, predominantly Native American and Hispanics who have a history of, or who are at risk for, substance use disorders. The overall goals of the four-year implementation project period are to (1) expand the delivery of substance use disorder-related healthcare services to include new and enhanced services; (2) deliver healthcare services through a strong community consortium; (3) utilize community engagement and evidence-based or innovative, evidence-informed model(s) in the delivery of substance use disorder-related health care services; and (4) improve population health and demonstrate health outcomes and sustainability.


Duties and Responsibilities of the Street Outreach Coordinator: (Included, but not limited to)

You'll report to the Director of Prevention and Recovery and:

  • Oversees coordination of outreach in accordance with the HRSA Rural Health Care Services Outreach Project grant and related outreach initiatives.
  • Implements outreach initiatives through the utilization of community engagement and evidence-based or innovative, evidence-informed models.
  • Coordinates and co-facilitates meetings with consortium partners.
  • Works with HR Administrator and Program Director to recruit and retain staff.
  • Administrative supervision of a team of Case Managers specific to outreach initiatives.
  • Along with Program Director and Operations Manager, implements and maintain systems of operation, including technology, vehicles, and supplies related to Case Management programs.
  • Gathers and maintains accurate data and records of services provided, including but not limited to Electronic Medical Records (EMR), intake documents, and other records of demographic information and services provided.
  • Works closely with the HRSA Grant Evaluator to gather, analyze and report data as required.
  • Represents the organization in community networking efforts and works to improve community relations.
  • Works with Program Director and Clinical Staff to establish and maintain a client-centered approach in outreach efforts.
  • Helps to promote and maintain a culturally responsive environment by effectively working with individuals or groups representing diverse backgrounds, interests, and points of view.
  • Communicates in a calm, respectful, and professional manner with clients and staff.
  • Maintains ethical and confidentiality rules and regulations in accordance with SFRC policies and procedures and HIPAA and 42CFR Part 2.
  • Promotes teamwork among staff, Consortium partners, funders, and community stakeholders.
  • Attends and co-facilitates HRSA Outreach Grant Technical Support meetings and other meetings and training relevant to street outreach initiatives and the HRSA grant.
  • Compiles information for Grant reporting as required.
  • Assists Billing Specialists with ensuring accurate billing for services.
  • Works with a medical provider, referral network, and intake coordinator to provide referral sources with information, paperwork, etc. as necessary to facilitate admission.
  • Works with community referral sources to generate referrals and increase and improve interagency collaboration including hospitals, other treatment providers, community stakeholders, Pueblos and Tribes, and others.
  • Other duties as assigned.
Requirements:


Street Outreach Coordinator Requirements

Education and Experience:

  1. Master's degree and clinical licensure preferred.
  2. Experience in human services, preferably in detox and rehab facilities, social services, or related Human Services fields.
  3. Excellent project management skills.
  4. Excellent verbal, written, and interpersonal skills.
  5. Demonstrated leadership skills and/or history of management.
  6. Detail-oriented and organized.
  7. Ability to problem solve.
  8. Familiarity with other community-based organizations and continuum of care.
  9. Works well with diverse populations.
  10. Must have and maintain a current, valid NM Driver's License. Must also have and maintain a clean driving record, as well as auto insurance.
