Job Details:

• Location: Richmond, KY
• Company: Sunbelt Staffing, on behalf of a Nationwide Home Health Agency
• Position Type: Contract, 13-week term with possibility of extension upon successful completion
• Schedule: Monday-Friday, 8:00 AM - 5:00 PM with occasional weekends and on-call
• Compensation: Competitive salary, mileage reimbursement, medical, dental, vision, and 401k benefits after 90 days

Job Responsibilities:

• Conduct home health visits to patients within Richmond, KY, and its surrounding areas, ensuring the delivery of high-quality care in the comfort of their homes.
• Perform comprehensive assessments of patients' health status, taking into account their individual needs and circumstances.
• Develop personalized care plans tailored to address the specific needs and goals of each patient, in collaboration with the healthcare team and under physician guidance.
• Provide skilled nursing care, including wound management, medication administration, patient education, and support for chronic disease management.
• Document all patient care activities accurately and promptly using Oasis and Homecare Homebase software platforms, adhering to agency protocols and regulatory requirements.
• Collaborate closely with interdisciplinary team members to facilitate coordinated care delivery and optimize patient outcomes.
• Maintain open communication with patients, families, and caregivers, offering support, education, and guidance to enhance their understanding and engagement in the care process.

Job Requirements:

• Active KentuckyRegistered Nurse (RN) License.
• Active American Heart Association (AHA) or American Red Cross Basic Life Support (BLS) certification.
• Minimum of 1 year of experience in home health nursing.
• Minimum of 1 year of experience with Oasis and Homecare Homebase software.
• Excellent clinical assessment and critical thinking skills.
• Strong communication and interpersonal abilities.
• Reliable transportation and willingness to travel within assigned territory.
