The Clinic Operations Manager will play a crucial role in ensuring the efficient and effective functioning of the clinic. This role involves overseeing various administrative and operational aspects, managing staff, and collaborating with healthcare professionals to maintain the highest standards of patient care. While also serving as our Chronic Care Manager they will be collaborating with healthcare teams, patients, and their families to develop and implement care plans that address the unique needs of individuals with chronic conditions. They will have a strong background in healthcare management, excellent organizational skills, and a passion for providing exceptional patient experiences. This position will report to the Director of Operations.

Supervisor Responsibilities:

  • Provide leadership oversight for all HHWC clinic operations.
  • Helping with budget development efforts for all HHWC clinic operations ensuring HHWC clinic operations operates in accordance with State, Health Plan and Federal clinic statutes/operating guidelines.
  • Providing global oversight for all system Nursing personnel.
  • Ensuring proper clinical support staffing for all system clinician practices.
  • Ensuring clinical support staff are trained how to properly support our system clinician medical practices.
  • Responsible for the fiscal, operational, clinical and regulatory compliance oversight of system clinic operations.
  • Designs, implements and monitors clinical support staff core clinical competencies.
  • Ensure all system clinician practices are staffed appropriately to maximize their practice operating efficiency/effectiveness.
  • Utilizes independent leadership judgement to drive elevated and ongoing continuous clinic operating performance improvement efforts.
  • Assesses and helps implement business analytics designed to drive elevated clinic operational performance.
  • Interfaces with clinicians to drive elevated clinician/clinical support staff team alignment.

Key Responsibilities:

    • Conduct comprehensive assessments of patients with chronic conditions to identify their healthcare needs, functional status, and potential barriers to self-management.
    • Evaluate and analyze patient medical history, current medications, and social determinants of health.
    • Develop individualized care plans in collaboration with the patient, family, and healthcare team, focusing on holistic and patient-centered approaches.
    • Coordinate and facilitate communication among healthcare providers to ensure seamless and integrated care delivery.
    • Educate patients and their families on the nature of chronic conditions, treatment plans, medication management, and lifestyle modifications.
    • Provide ongoing support and resources to empower patients in self-management of their chronic conditions.
    • Implement strategies for monitoring and tracking patient progress, including regular follow-up appointments, lab tests, and medication adherence.
    • Utilize data and analytics to identify trends and areas for improvement in patient outcomes.
    • Collaborate with physicians, specialists, pharmacists, social workers, and other healthcare professionals to ensure a coordinated and multidisciplinary approach to care.
    • Communicate effectively with patients, families, and caregivers to address concerns, answer questions, and provide guidance.
    • Participate in quality improvement initiatives to enhance the overall effectiveness and efficiency of chronic care management programs.
    • Stay abreast of evidence-based practices and emerging trends in chronic disease management.
    • Maintain accurate and up-to-date documentation of patient assessments, care plans, interventions, and outcomes.
    • Generate reports and summaries to evaluate program effectiveness and identify areas for improvement.
Required Skills/Abilities:

  • Strong problem-solving and analytical skills.

  • Excellent communication and interpersonal skills.
  • Proven experience in clinic or healthcare management.
  • Strong knowledge of healthcare regulations and compliance requirements.

  • Ability to work collaboratively in a team-oriented healthcare environment.
  • Proficient in using electronic health record (EHR) systems.
  • Commitment to continuing medical education and professional development.

Education and Experience:

  • Active and unrestricted Registered Nurse (RN) license.
  • Bachelor of Science in Nursing (BSN) required; Masters preferred.
  • 5+ years of leading/directly overseeing multiple complex medical practices.
  • Refined acumen developing/analyzing and utilizing business analytics to assess and continually improve clinic operating performance
  • Certification in chronic care management or related field is a plus.

Physical Requirements:

  • Prolonged periods of sitting at a desk and working on a computer and standing, bending and moving.
  • Must be able to lift up to 15 pounds at times.
