QUALIFICATIONS: Highschool Diploma required, Associate or Bachelor’s degree preferred. 3-5 years of experience in a healthcare setting required. Must have knowledge of the recovery journey and be able to maintain and teach healthy boundaries. Prior social work, care management or experience working with MAT (Medical Assisted Treatment), OBOT (Office-Based Opioid Treatment) or similar program preferred. At least two years of sobriety required.

PURPOSE: The MAT Case Manager will be an integral part of the Recovery Services team responsible for working with the patient, family, and healthcare team to coordinate resources and services. The MAT Case Manager will support the patient through recovery of substance use disorder (SUD). Minimum of 2 years of sobriety/clean time if in recovery.

ADA ESSENTIAL FUNCTIONS:

  • Hearing: within normal limits with or without use of corrective hearing devices.
  • Vision: adequate to read 12-point type with or without use of corrective lenses.
  • Must be able to verbally interact with staff, clients and public.
  • Manual dexterity of hands/fingers for writing, computer input.
  • Able to lift up to 40 lbs.
  • Standing 25-75% of the day.
  • Walking 25% of the day.
  • Pushing up to 30 lbs.
  • Pulling up to 20 lbs.

RESPONSIBILITIES:

  • Maintains a positive peer relationship and performs as a team player.
  • Plans and prioritizes to maintain a time and attendance record which complies with company policy.
  • Provides excellent internal and external customer service assistance, providing knowledgeable and appropriate information to customers.
  • Works independently in a very detail oriented manner and meets deadlines.
  • Employee reports to work in a timely manner.
  • Employee utilizes breaks and meal periods to care for business outside of the work area
  • Employee appropriately utilizes time and attendance for clocking in and out and schedules absences in advance when possible.
  • Employee communicates absence to supervisor in a timely manner.
  • Be the point of contact for MAT patients within the medical department
  • Assist with program assessment/intake of eligible patients with Behavior Consultants.
  • Schedule eligible MAT patients with assigned medical providers.
  • Obtain and review ROI’s/consents/treatment contracts for MAT patients and their support members.
  • Support patient with navigating their individual service plans/treatment plans in supporting their ability to comply with treatment goals.
  • Manage and monitor patients during induction, stabilization and maintenance phases along with assistance from recovery coach as needed
  • Track and report on patient progress and compliance.
  • Assist in monitoring relapse/exacerbations and triage concerns to clinical staff.
  • Obtain and review ROI’s/consents/treatment contracts with patients and their significant others/support members.
    • Work with Community Health on patient transports to and from detox/inpatient care settings.
    • Be able to assist driver with patient transports.
    • Responsible for tracking inventory, ordering, and stocking necessary items for MAT patients.
  • Coordinates with Wellbriety program for assistance with housing, employment, transportation, and other as-needed services and resources.
  • Assist with data collection and tracking for MAT related grants.
  • Completes reporting and updates as requested.
