Job Description

The Contracted Targeted Case Manager (TCM) is highly motivated to make a difference in the community and committed to the well-being of their clients. The TCM will utilize experience and knowledge to assess needs of clients and their families in order to link, coordinate, and assist in implementing the appropriate resources. They are responsible for managing an assigned caseload and responding appropriately to client needs in compliance with agency, licensing, and Medicaid standards. This includes, but is not limited to: assessing client needs; developing, implementing and reviewing service plans; and working with other community resources in achieving client service needs. Caseloads may include diverse populations with a variety of clinical needs. The Targeted Case Manager is a positive team player that promotes a culture of inclusion and participation.

THE AGENCY AGREES:

  1. To provide a licensed professional person to oversee the Contractor and the caseload assigned to them;
  1. To provide the Contractor with information and reasonable guidance or assistance with the work s/he is providing for the Agency;
  1. To provide the Contractor with as much information about the client and family as necessary to fulfill the responsibilities in providing service to the client and family;
  2. To provide payment for services to the Contractor each month end based on a $200 fee/billable client for services rendered upon submission of complete and proper documentation of services;
  3. To provide reimbursement for miles driven in the course of their work at a rate of $.32 per mile outside of Jessamine County ;
  4. To provide $200 upon completion of Targeted Case Management Training certification (12 hours);
  5. To provide $100/each upon completion of each of the following TCM modules:
  • Six (6) hour Substance Use Disorder Targeted Case Management Training
  • Six (6) hour Severe Emotional Disability Targeted Case Management Training
  • Six (6) hour Severe Mental Illness Targeted Case Management Training
  1. To provide $100 upon completion of New Case Manager training checklist; and  $20 per hour for attendance at Treatment Team Meetings and other meetings as specified by the Director. (Treatment Team Meetings are typically conducted twice monthly and are approx. two hours in duration.)
  1. To pay Contractor for services provided within 30 days of receipt of invoice when submitted within the appropriate timeframe and satisfactory completion (per the Director) of services and documentation provided at $200 per billable client.

Payment will not be made for missed or cancelled appointments. Payment will not be rendered until services are delivered and all required paperwork and/or report(s) is/are received. The Agency reserves the right to refuse payment for any report or service delivered outside the evaluation timelines outlined in this agreement or as a result of negative professional behavior which is outside of the standards of practice /expectations included as a part of this agreement.

It is understood and agreed that the Contractor is not an employee of the Agency and is therefore not eligible for any “fringe benefits” except as specified herein. Additionally, as it is understood that the Contractor may not be protected for malpractice with the agency’s insurance.

Education & Professional Requirements:
  • Bachelor’s Degree from an accredited institution in a human services field with a minimum of 1 year of experience in a behavioral health setting post-bachelor’s degree.
  • Certification in the following Department For Behavioral Health, Developmental and Intellectual Disabilities approved trainings to meet the requirements outlined in Kentucky State Regulations and Statutes:
    • Ten (10) hour Community Support Associate Training
    • Twelve (12) hour Targeted Case Management Core Training
    • Six (6) hour Substance Use Disorder Targeted Case Management Training
    • Six (6) hour Severe Emotional Disability Targeted Case Management Training
    • Six (6) hour Severe Mental Illness Targeted Case Management Training

Job Specifications

  • Must be at least 21 years of age.
  • Valid driver’s license, reliable transportation, and proof of current auto insurance.
  • Valid certification in CPR and First Aid (can be provided upon hire).
  • Must submit to required background checks and pre-employment drug screening.
  • Effective communication skills including proper use of agency communication systems.
  • Strong interpersonal and engagement skills.
  • Demonstrates professional maturity and respect for others.
  • Strong organizational and time-management skills.
  • Strong documentation knowledge and skills required.
  • Strong computer skills; experience using EMR system preferred.
  • Ability to support the agency’s mission and philosophy, and demonstrate respect for cultural diversity and team-centric harmony.
