Clinic Patient Care Coordinator

Description:Position Summary

Reporting to the Director of Population Health, the patient care coordinator supports patients and clinic staff in the depart and referral processes for all clinic patients. This is a highly visible position that interacts with patients and their families along with hospital staff both internally and at all referral facilities to coordinate access to needed services/specialists. The patient care coordinator must effectively deal with challenges and conflict working with the patient, family and/or caregivers, and the multidisciplinary care team.

Accountabilities

• Schedule any laboratory and radiology needs stemming from the provider visit and departing the patient when the visit in complete with any needed follow-up appointments.
• Begins any needed prior authorization for additional care ordered by the provider and schedules the patient for the needed services once the authorization is complete.
• Works with outside offices/hospitals to obtain timely appointments for services needed and not offered at Gothenburg Health whiles gathering and sending any needed information to the accepting facility.
• Updates patient information as obtained and completes documentation according to identified process and workflows for consistent communication and care.

Minimum Qualifications

• High School diploma or GED
