Job Description
Hospice Social Worker – BSW / MSW
HealthCare Support is actively seeking a Hospice Social Worker – BSW / MSW to fill an opening with a Large Healthcare Network in West Palm Beach, FL.
Daily Responsibilities for Hospice Social Worker – BSW / MSW:
- The Social Worker will support patients and families going through Hospice care
- Daily responsibilities include assessing needs, collaborating with the medical team, designing proper plan of care, and ongoing communication with assigned cases to work toward goals
- 8-30am – 5pm
- Sunday - Thursday OR Tuesday - Saturday
Required Qualifications for Hospice Social Worker – BSW / MSW:
- Graduate of a Masters or Bachelors program – accredited by the Council on Social Work Education
- At least one year of experience in social after degree completion
- Experience in the medical setting as a social worker
Preferred Qualifications for Hospice Social Worker – BSW / MSW:
- Hospice expereince
