Job Duties:

·       Companionship

·       Medication Reminders

·       Personal Care

·       Transportation for errands/appointments

·       HOSPICE Care/Support

·       Support Wellness & Home Health

·       Assist with Adult Daily Living (ADLs) activities

·       Mobility Assistances and Transfers

·       Take accurate, daily notes, of clients’ activities and baseline conditions
