Licensed Practical Nurse-Home Care (PRN)

American Hearts and Home is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our dedicated team in providing exceptional home care services to our clients. Job Responsibilities:

• Provide comprehensive nursing care to patients in their homes according to individualized care plans.
• Administer medications, treatments, and therapies as prescribed by physicians.
• Monitor patient's vital signs and condition, and report any changes to the supervising Registered Nurse (RN) or physician.
  
• Educate patients and their families on disease management, medication administration, and proper care techniques.
• Maintain accurate and up-to-date patient records and documentation.
• Collaborate with other healthcare professionals to ensure coordinated and effective care delivery.
• Adhere to all relevant nursing standards, regulations, and ethical codes of conduct.

Qualifications:

• Valid LPN license in the state of GA
• Minimum of 2 years of experience in nursing, preferably in home care or a similar setting.
• CPR certification required.
• TB Test required
• Strong communication and interpersonal skills.
• Ability to work independently and as part of a multidisciplinary team.
• Compassionate and patient-centered approach to care.
• Reliable transportation and willingness to travel to patients' homes within the designated area.
• Flexibility to work evenings, weekends, and holidays as needed.
