Job Description

Job Title: SOS Case Manager

 

Summary of Job Duties: The Case Manager is responsible for assessing, coordinating and providing services for women who are admitted to the program. 

 

Qualifications:  To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.  This position requires a HS diploma and 2 years job related experience.  Bachelor degree in related field is preferred. Knowledge of women’s issues in recovery is helpful.  If in recovery, a minimum of 2 years continuous abstinence from drugs/alcohol required.  Submit to and pass a criminal record check and a drug screening test.

 

Hours: Monday through Friday 8:00 am until 4:00 pm. Evening and weekend hours may be required.

 

This position reports directly to the Cumberland River RHOAR Program Director.

 

Essential Duties and Responsibilities:

Include the following:

 

  • In coordination with the Program Director, plans, coordinates, and evaluates all programmatic and administrative needs and responsibilities of clients.
  • Assess client employment and job training needs.
  • Teach open classes to address job/interview skills.
  • Follow up with clients and employers to ensure successful job placement.
  • Prepare and maintain a list if agencies and contact information for referrals.
  • Maintain and submit food stamp applications for clients.
  • Coordinate with parties involving the food stamp cards of clients who have left the program.
  • Attend Community.
  • Coordinate with courts, judges and referral agencies to provide services.
  • Works regionally with police, medical personnel, and other community.
  • Service agencies and individuals to provide and promote optimal services to clients.
  • Screens all clients for admission and maintains a waiting list.
  • Provides daily supervision to ensure accurate, complete, and up-to-date records are kept and all clientele are receiving appropriate care.
  • May be required to perform drug screenings on clients as needed.
  • Facilitates communication with other Recovery Kentucky Program staff.
  • Attends and participates in all required meetings.
  • Demonstrate the ability to work effectively with other team members.
  • Other duties as designated by Program Director.

Environmental Data and Job Hazards:

  • Hours of work are structured 8.0 hour blocks of time but may be subject to irregular and/or flexible hours based on needs or demands
  • Position may require duties to include routine or reasonable anticipated tasks or procedures where there is a degree of actual or potential exposure to blood or other infectious materials.
  • All staff are required to participate in yearly trainings regarding infection control and safety in the environment including hazardous waste and blood borne pathogens.
  • All new staff must attend training regarding handling physical, verbal threats, acts of violence or other escalating and potential dangerous situations including when police need to be summoned.
  • All staff must be certified in Cardiopulmonary Resuscitation within one month of employment and maintain the certification, if providing direct care.
