POSITION SUMMARY - RN Case Manager
POSITION DUTIES Discharge Planning / Utilization Review
PREFERRED QUALIFICATIONS RN Care Manager for discharge planning and care coordination, requires prior acute care discharge planning experience
LENGTH OF ASSIGNMENT 13 weeks
SHIFT / HOURS PER WEEK Days 8-4 / 40
START DATE 4/25/24
