Job Description
North Homes Children and Family Services is a private, non-profit agency that provides comprehensive mental health services to children and families across Northern Minnesota. Our mission is to provide a community-based continuum of compassionate care to children and families. Our adult services program is growing and we are looking for full-time Adult Targeted Case Managers who want to be part of our growth! Case Managers assist adults with serious and persistent mental illness in gaining access to needed medical, social, educations, vocational, and other necessary services as they relate to the client’s mental health needs. Case Management services include developing a functional assessment, an individual community support plan, referring and assisting the person to obtain needed mental health and other services, ensuring coordination of services, and monitoring the delivery of services.
Coordination / Communication
- Develop, maintain and utilize both written and verbal communication skills.
- Handle various aspects of service coordination and communication with respects to the program.
- Work with social workers, probation officers and other outside agencies in a professional and responsible manner on behalf of the client and family.
Client Interaction
- Adhere to confidentiality and mandatory reporting laws.
- Implement goals and procedures for the client as indicated on the individual community support plan prepared by the case manager or goals and procedures developed by the Clinical Supervisor during clinical supervision.
- Provide assistance to gain access to services needed related to client’s mental health needs.
- Identify current strengths and needs relevant to the client and their situation.
Documentation and Training
- Complete all paperwork in an appropriate, accurate and timely manner and according to agency guidelines.
- Attend all continuing education and training, as mandated by the agency or licensor.
All Other Duties As Assigned
Other duties may be assigned to this position periodically depending on the needs of the agency and/or Human Resources Director.

Requirements:A case manager must:
(1) be skilled in the process of identifying and assessing a wide range of client needs;
(2) be knowledgeable about local community resources and how to use those resources for the benefit of the client;
(3) have a bachelor's degree in one of the behavioral sciences or related fields including, but not limited to, social work, psychology, or nursing from an accredited college or university; and
(4) meet the supervision and continuing education requirements.
