Job Description
Are you an all-nighter kind of person? Maybe enjoy a really clean home. We could use your night owl support in the Supported living homes. We provide services to individuals on the DD waiver. In this setting, you will be assisting individuals with personal care at some locations, personal grooming/hygiene, medication delivery, cleaning the common areas of the home, preparing breakfast, packing lunches for their daytime outings, and transporting individuals to their daytime meeting locations.

Graveyard positions are completely awake positions. There are many nights when individuals are up and down all night and your attention to them is needed.

Differential pay for residential supported living shifts is $2.00/hour on top of base pay
Available Shifts:
202- Friday 11 pm - 9 am, Saturday & Sunday 9 pm - 9 am (34 hours)
204 - Saturday & Sunday 9 pm- 9 am, & Monday 11 pm - 9 am (34 hours)
205 - Thursday & Friday 11 pm - 9 am, Saturday 9 pm - 9 am (32 hours)
202- Wednesday & Thursday 11 pm - 9 am ( 20 hours)
All employees can pick up additional shifts when available and are fully trained in each home.
Position Duties and Responsibilities
- Contribute to the evaluation of the individual’s progress and provide input to the team concerning the development of appropriate plans to achieve specific ISP outcomes.
- Complete written and verbal reports/ documentation related to daily progress.
- Act as a positive role model to individuals served.
- Provide life skills support and behavior management
- Assist with hygiene, grooming, assist with medication delivery, housekeeping, laundry, cooking, and any other areas identified within the ISP.
- Provide transportation (using a personal vehicle) to the individual(s) served to access the community.
- Identify social, economic, recreational, and educational services in the community that will meet the individual’s needs.
- Provide emotional support and feedback to the individual and their family.
- Attend all house meetings, complete all required training, and work within all HeartWell Policies & Procedures.
Position Requirements
- Minimum – High school diploma or equivalent
- Current and Valid Driver’s License
- Safe insured/ registered vehicle
- Ability to lift 50lbs
