Admissions Hospice Registered Nurse - 208BHR

Envision Healthcare at Home was established in 2005 by six healthcare professionals who wanted to make a difference in their community.

We are currently seeking a Full-time, Admissions – Hospice Registered Nurse (RN) who is passionate about providing quality care to patients and families within the South King County (south of Seattle), Pierce County, Snohomish County, and Thurston County area.

This position's schedule is 4 shifts per week at 10 hours and the days would be Wednesday through Saturday.  On-call will include 1x per month for 1 weekend to help with patient coverage.

Admission RN responsibilities include:

• The primary job is to Admit new referrals into our care for Hospice.
• Coordinating all aspects of the interdisciplinary care and services provided to hospice patients and their families.
• Provides education to families and caregivers.
• Develops, implements, and reviews healthcare plans.
• Advocating for patients and their families
• Documentation of care
• Symptom management
• Promoting comfort and quality of life.

Job Requirements:

• Current RN license to practice in the State of WA.
• Current WA driver’s license and reliable transportation
• Maintain current CPR certification.
• Must have at least a year of Hospice Case Management experience. 
• Demonstrate excellent observation, problem-solving, verbal, and written communication skills, and nursing skills per the competency checklist.
• Shows ability to organize and prioritize workload independently.
