Position Overview:

Partners with Sutter Health patients in unique populations (oncology, bariatric, spinal trauma, stroke, etc.) and guides them through a comprehensive array of health services in accordance with established policies, procedures, protocols, techniques, and guidelines. Identifies and works to resolve barriers to care, assists with access issues, develops relationships with health care providers, explains insurance and payment systems, and coordinates services with providers, patients, and caregivers within Sutter Health, at external facilities, and in the community to provide seamless and integrated quality care. Raises awareness about healthcare organizations and agencies information resources, Sutter Health support programs and services, community resources, clinical trials, and helps to meet other illness-related needs identified by the patient, their family, or the patient's support group. Assists with ongoing evaluation and quality improvement of the program, including community outreach and marketing.

Job Description:

EDUCATION:
Equivalent experience will be accepted in lieu of the required degree or diploma.

  • Bachelor's: Social Work, Health Education or related field or equivalent education/experience
  • OR Other: Graduate of an accredited school of nursing.


CERTIFICATION & LICENSURE:

  • RN-Registered Nurse of California
  • OR LCSW-Licensed Clinical Social Worker
  • OR RCP-Respiratory Care Practitioner
  • OR PT-Physical Therapist within 1 Year of hire
  • OR OT-Occupational Therapist


Depending on focus area, possess current breast patient navigator certification or attainment of certification within the first calendar year of work.


TYPICAL EXPERIENCE:

  • 5 years of recent relevant experience.



SKILLS AND KNOWLEDGE:

  • Demonstrated competency in applying evidence based knowledge, theory, practices, principles, and processes of professional nursing.
  • In-depth knowledge of focus area, including diagnosis, current treatment options, complementary therapies, potential complications, family/support group dynamics and associate quality of life issues.
  • Working knowledge of community resources for the assigned focus area, including requirements and accessibility.
  • Thorough knowledge of healthcare systems, insurance/payment systems, and medical terminology.
  • Solid analytical and project management skills, including the ability to analyze problems, reach practical conclusions, and institute effective changes.
  • Possess written and verbal communications skills to explain clinical information and program data clearly and professionally to diverse audiences.
  • Computer proficiency—Microsoft Office Suite, familiarity with databases, and experience with electronic health record systems.
  • Work independently, as well as be part of a team, while demonstrating exceptional attention to detail and organizational skills.
  • Prioritize and delegate assignments under varied and urgent conditions while working within standardized procedures and practices to achieve objectives and meet deadlines.
  • Identify and solve complex problems based on the analysis of multiple sources of information while providing new perspectives on existing solutions.
  • Ensure the privacy of each patient’s protected health information (phi).
  • Build collaborative relationships with other healthcare providers (physicians, nurses, etc.), healthcare organizations and agencies, and community resources to coordinate efforts and provide the highest quality of patient care.

Pay Range is $42.98 to $64.47 / hour

The salary range for this role may vary above or below the posted range as determined by location. This range has not been adjusted for any specific geographic differential applicable by area where the position may be filled. Compensation takes into account several factors including but not limited to a candidate’s experience, education, skills, licensure and certifications, department equity, training and organizational needs. Base pay is just one piece of the total rewards program offered by Sutter Health. Eligible roles also qualify for a comprehensive benefits package.
