Company DescriptionHealthCare Support is actively seeking a Field RN Case Manager to fill an opening with a Fortune 500 Healthcare Organization in Toms River, NJ.
Daily Responsibilities for Field RN Case Manager:
- Ensure accurate and complete documentation, assist with benefit determination
- Travel to 3 locations (typically 1 Location per day)
- Monday – Friday 8am – 5pm
- Associate degree or higher in Nursing (RN)
- Active and unrestricted license in New Jersey
- Certified in Basic Life Support
- 1 year working in a Long-Term Care facility/ Med-surg/ ER experience as an RN
- COVID-19 Vaccine
Preferred Qualifications for Field RN Case Manager:
- 1+ year of clinical experience in practice (long-term care setting preferred)
- Experience in meeting the medical needs of patients with complex behavioral, social and/or functional needs
- Understanding of Geriatrics and Chronic Illness
