Job Description
Pathway Homes is a private, nonprofit striving to end homelessness by providing housing and services for adults with mental illness and other co-occurring disorders in Northern Virginia. We are opening an Assertive Community Treatment (ACT) Team that will be based in the Reston/Herndon areas of Fairfax County.
An ACT program is an interdisciplinary team that provides wrap around services 7 days per week to individuals in their homes and in the community. The goal of program is to decrease the utilization of ERs, in-patient hospitalizations and involvement with the legal system, by supporting individuals in developing effective healthy living skills.
We are seeking a part-time (16 hours/week) Physicians Assistant or Nurse Practitioner (NP).
The PA's Essential Duties & Responsibilities:
< Performs psychiatric and medical services to include evaluation, examination and diagnosis.
< Oversees medication management including the prescribing, administration and documentation.
< Completes health assessments and determines diagnostic and screening tests needed, orders and interprets laboratory results.
< Writes referrals to hospitals, specialists and other healthcare services as needed.
< Communicates and coordinates with other healthcare providers.
< Provides health promotion, prevention, and education, as well as assesses individual for medication reactions and side effects.
< Supports other team members in the observation and monitoring of symptoms.
< Works with individuals where they are at in their stage of change with respect to healthcare.
< Participates in the development, implementation, and monitoring of individualized service plans.
< Is a strong advocate for consumer self-determination and independence and honors day to day choice and supports the team members in supporting individuals in their recovery.
< Uses motivational interviewing and outreach techniques, cognitive behavioral approaches, and relapse prevention techniques to support program participants.
< Responds to crises and takes appropriate action to ensure the safety of participants and community.
Job Requirements: Must have active Physicians Assistant or NP license in the state of Virginia. Experience working with adults with mental illness and co-occurring disorders. Must have valid drivers license and reliable vehicle. Must be able to pass a driving record check and a criminal background check.
Pay rate $70-75 per hour based on experience. 16 hours per week.
