Person-Centered Counselor

The Minnesota River Area Agency on Aging is expanding its Veterans Directed Care program and seeking a full-time Person-Centered Counselor professional in the Luverne/Pipestone, MN area. This work-from-home position requires travel in the southwest Minnesota and southeast South Dakota areas.

Person-Centered Counselors help Veterans who need assistance choosing their caregivers, services and supports so they can safely live in their homes. Assess the functional status of the Veteran and identify areas of need and preferences to meet those needs. Develop and help implement a care plan that meets the Veterans' identified needs and does not exceed their budget allocation. Perform ongoing monitoring of services and support to meet the Veteran's needs.

This position requires a bachelor's degree in a human services-related field, such as social work, nursing, public administration or gerontology and two years of professional or internship experience working with older adults. A degree in social work requires licensure or demonstrated progress and intent on securing licensure. Applicants for this position must have one year of professional or internship experience in the social services field, including long-term care, health care, social services or lead agency setting. Candidates must be able to work from a home office in the Luverne/Pipestone, MN area.

The hourly range for this position is $22- $24 with great benefits, including participation in health, dental, life insurance, Accidental Death & Disability, and long-term disability insurance, subject to different benefit plans.
