The Position: Community Health Worker

Employment Status: Full Time

Hours per two-week pay-period: 40

Essential Job Functions:

• Provide Exceptional Customer Service Skills.
• Demonstrates successful problem-solving skills to provide assistance in care needs
• Develops relationships with teams to facilitate services for patients.

Minimum Qualifications:

• High Schools Diploma

Certifications and Licenses:

• Ability to obtain Community Health Worker Certification with in a year of employment.

Position Summary:

Sidney Health Center is seeking a Community Health Worker to join our I CARE team in the Care Management setting. Essential duties of the position include exceptional customer services and problem-solving skills along with developing team relationships to provide services for patient needs. If you enjoy serving others through purposeful work, Sidney Health Center is the place for you. The Care Management Team is critical to patient care in the community.
