If you share our commitment to providing personalized care and have a desire to make a difference, then come join us on this rewarding journey!Options Medical Weight Loss Nursing care coordinator will play a vital role in our telehealth by coordinating and managing the overall care of patients. Their primary responsibility is to ensure that patients receive appropriate and timely care throughout their healthcare journey through the virtual journey. This is a clinic-based position that will coordinate patient care with the telehealth team. Applicants should be within proximity to any of the Options Medical Weight Loss locations in Chicago, Columbus, Cleveland, Tampa, Atlanta, Detroit or Indianapolis. Position Responsibilities: Patient Assessment: Conduct comprehensive assessments of patients' medical conditions, health needs, and social factors to develop individualized care plans.Care Planning: Collaborate with healthcare providers, including physicians, nurses, and allied health professionals, to develop and implement care plans that address patients' physical, emotional, and social needs.Care Coordination: Serve as the central point of contact for patients and healthcare team members to coordinate and facilitate the delivery of care services. Ensure effective communication and information sharing among all parties involved.Patient Advocacy: Advocate for patients' rights, preferences ensuring that their needs are respected in decision-making processes that falls within the scope of practice of weight loss and obesity medicine.Resource Management: Assess and coordinate necessary resources, such as medical equipment, medications, and telehealth services, to support patients' care plans.Education and Support: Provide patients with education and support regarding their medical conditions, treatment plans and self-care plans. Instruct patients on self-injection via telemedicine appointments.Monitoring and Evaluation: Regularly assess patients' progress and modify care plans as needed. Monitor patients' adherence to treatment plans and provide interventions to address any barriers or challenges.Documentation: Maintain accurate and up-to-date records of patients' medical history, assessments, care plans, and interventions in accordance with healthcare regulations and policies.Quality Improvement: Participate in quality improvement initiatives, such as identifying areas for improvement in care delivery, developing and implementing evidence-based practices, and monitoring outcomes.Collaboration and Teamwork: Collaborate with interdisciplinary healthcare teams to ensure seamless coordination of care at home via our telehealth platforms.

Knowledge, Skills and Abilities: Education and Experience - Education: A bachelor's degree in nursing (BSN).Licensure: Valid and unrestricted nursing license.Experience: Previous experience in nursing care coordination or medical case management.Knowledge: Strong knowledge of nursing principles, care coordination practices, and healthcare regulations.Familiarity with electronic health record (EHR) systems and proficient technological literacy is required.Familiarity with telemedicine or a willingness to learn and participate in telemedicine workflows.
