Qualifications:

1. Graduate of an approved CNA program, holding a certification in the State of Virginia.

2. Experienced in Home Health, private duty, hospice, skilled nursing or long-term care, or Assisted Living.

3. Must be able to lift up to 50lbs and perform all physical requirements of patient care.

4. Must be able to pull, push, squat and bend.

5. Must be compassionate and interested in providing direct patient care

6. Current drivers license & car insurance.


Job Knowledge:

1. Possesses a working knowledge and understanding of the functional organizational make-up of a Home Health/Hospice Agency.

2. Working knowledge of State, Federal and ACHC guidelines/regulations (will train on the job.)

3. Working knowledge of general computer programs (ie: Word, Excel, electronic medical record charting.)

  1. Basic computer usage, internet, typing, emailing, and word processing skills.

4. Working knowledge of general medical terminology.


Responsibilities:

1. Assistance with activities of daily living, including bathing, showering, and personal care

2. Light house keeping

3. Laundry

4. Companion care

5. Errands

6. Home maker

7. Works closely with the Administrator and Director of Nursing for Home Health Care and Hospice Services.

8. Keeps the Administrator and Director of Nursing apprised of updates and conflicts on a daily basis or as needed.

9. Acts as a liaison among Agency personnel, patients, and family members when necessary.

10. Maintains a professional business relationship with the medical community and other health care entities.

11. Acts as a good steward of resources, implementing innovative process improving techniques.

12. Adheres to agency policies and procedures.

13. Performs other duties as assigned. Must be adaptable and able to think outside the box, forward thinking, and willing to jump in and help!
