Registered Nurse Case Manager

Ancora Home Health and Hospice is currently seeking a compassionate Registered Nurse (RN) that has a passion for providing outstanding services to the elderly in senior living communities and in-home health settings. This is a full-time position serving the Wasilla area.

Each Registered Nurse is an important part of the Aegis Health Care team. Job Responsibilities:

  • Perform appropriate assessment of physical, social, and psychological status
  • Use assessment information and critical thinking skills to develop an individualized plan of care; revise and reassess changes as appropriate
  • Actively seek patient, family, team, and physician involvement to develop plan of care including needed education
  • Set priorities, coordinate, and organize patient care delivery through effectively managing time, supplies, and resources
  • Initiate referrals as needed to other health team members
  • Maintain patient records showing systematic assessment, planning intervention, and evaluation
  • Participate in team meetings and coordinate patient care with other members of the team

Skills and Qualifications:

  • Current Registered Nurse (RN) license in state of practice
  • Home Health / Hospice experience as a Registered Nurse (RN) preferred
  • Ability to utilize full range of movement to transfer patients
  • Ability to communicate effectively both verbally and in writing
  • Valid Driver's license, clean driving record, and reliable automobile
  • Proof of current automobile insurance
  • Criminal background check clearance/fingerprint clearance
  • Current CPR certification
  • Negative TB test

RN Case Manager Job Description:

The registered nurse plans, organizes and directs home health and hospice care and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities.

Additional Duties:

Participates in on-call duties as defined by the on-call policy.

Ensures that arrangements for equipment and other necessary items and services are available.

Supervises ancillary personnel and delegates responsibilities when required.

Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes.

Fulfills the obligation of requested and/or accepted case assignments.

Actively participates in quality assessment performance improvement teams and activities.

Job Factors:

Physical Requirements:

Requires considerable physical effort most of the day including kneeling, squatting, reaching, twisting, climbing, walking, exposure to temperature and humidity changes and maximal assist in lifting and/or transferring of a 100-pound patient. Must possess sight/hearing senses or use appropriate adaptive devices that will enable senses to function at a level required to meet essential duties of this position. Must provide evidence of annual TB test and other state required tests or examinations.

Mental Requirements:

Must be able to work independently, make judgments based on assessments and data available and act accordingly. Must be flexible, innovative and possess good interpersonal skills. Must be able to cope with mental and emotional stress and demonstrate emotional stability.

Working Conditions:

Be able to tolerate exposure to elements including, but not limited to, odors, blood, body fluids and excrements, adverse environmental conditions and hazardous materials.
