P.A.C.E. offers a wide variety of services to fulfill this mission such as medical services to medications, from nutritious meals to transportation – all personalized to meet the participant’s specific needs!


The Home Care Coordinator LPN orchestrates the delivery of health care services for PACE participants in all care settings according to their plan of care.

QUALIFICATIONS:

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill and/or ability required. 1. Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.

2. One or more years working with the frail and elderly population.

3. Must have a valid driver’s license, proof of insurance and have means of transportation. Must be able to meet auto insurance carrier's standards for coverage.

• Education: Graduate of a school of professional nursing. Current North Carolina Licensed Practical Nurse (LPN) License, Cardiopulmonary Resuscitation (CPR) certification required.

• Experience: Minimum of two years of experience providing skilled nursing care.

One year of community health nursing/home care agency experience preferred. Experience in performing comprehensive geriatric assessments preferred.
