What You Should Know About the LPN Float Role:
  • The LPN Care Manager position is a driving position.
  • This is a Full-Time Days position M-F, 8:00A-4:30P; 40 hours/week.
  • We currently have two openings: One will serve the Marysville/Columbus region and the other will support Morrow County.
  • We provide superior care and superior services to patients at their end-of-life journey. Only those who have a heart for hospice will succeed.

The LPN Float Essential Duties Are:

We provide superior care for patients in families in homes/facilities. The LPN Care Manager will provide quality nursing care to patients in a manner that meets the patient and family needs, working under the direction and supervision of an RN. You could provide superior clinical care to patients and their families.

Qualifications:

  • LPN with the State of Ohio without any board actions.
  • One year of nursing experience preferred.
  • CPR Certified.
  • Computer skills sufficient to properly document services and care.
  • Ability to drive during daytime, nighttime or inclement weather.
  • Valid Driver's License with Safe Driving Record.
  • State Minimum Automobile Insurance Coverage.
