Patriot Homecare has immediate opening's in Knoxville TN and surrounding areas for FULL TIME and PART TIME day shift positions.

SUMMARY: Helping families and individuals restore balance, order, and peace to their lives and help their loved ones remain at home. Assist client with activities of daily living and personal care.

JOB RESPONSIBLITIES & DUTIES:

• Assist with ADLs.
• Check and record vital signs daily.
• Care for individuals or families during periods of incapacitation, family disruption, or convalescence, providing companionship, personal care, or help adjusting to new lifestyles.
• Administer bedside personal care, such as ambulation or personal hygiene assistance.
• Prepare and maintain records of client progress and services performed, reporting changes in client condition to supervisor.
• Plan, shop for, or prepare nutritious meals based on client’s nutritional requirements.
• Participate in case reviews, consulting with the team caring for the client, to evaluate the client’s needs and plan for continuing services.
• Instructor advise clients on issues such as household cleanliness to prevent infection, utilities, hygiene, or nutrition.
• Provide clients with communication assistance, typing their correspondence, or obtaining information for them regarding their care.
• Transport clients to locations outside the home, such as physician’s offices or outings that assist them with their activities of daily living.
• Assist family and instruct them on how to provide care during unstaffed hours.
• Be prompt to scheduled visits and notify the supervisor immediately when unable to do so.
• Be familiar with all rules, policies, and procedures in providing services.
• Submit all required documentation in a timely manner.
• Follow the individual service recipient’s plan of care.
• Keep all records and names of service recipients confidential unless authorized by recipient’s written consent.
• Immediately report any incident of injury to supervisor.
• Immediately report any suspicion of abuse or neglect t to appropriate authorities and the supervisor.
