Performs nursing duties and provides direct patient care under the direction and supervision of a Registered Nurse who is responsible for the care, comfort, safety and well-being of assigned patients. Documents care provided appropriately in patient chart according to policy and procedure.

Assists the RN in the assessment of the patients' needs in conjunction with the individualized plan of care for each patient.

Performs plan of care interventions, including specimen collection and wound care. Administer medication accurately in compliance with hospital policy and procedure. Documents patient care given.

Assists patients with performing activities of daily living, including personal hygiene, elimination, nutrition, and ambulation.

Monitors, documents, and communicates patient's status and reactions to interventions with RN, physician, patient, and families.

Provides care that reflects the patient care plan and evaluates the patient's progress toward attainment of outcomes.

Assists in patient/patient representative teaching (re: knowledge deficit of treatment plan for current health problems) as directed.

Works as an advocate for the physical and emotional well-being of the patient.

Maintains confidentiality and recognizes patients' rights.

Demonstrates understanding of importance of Standard precautions and isolation techniques.

Demonstrates knowledge of aseptic technique while performing procedures.

Demonstrates knowledge of patient care needs regarding pediatric patient (0-12 years of age).

Demonstrates knowledge of patient care needs regarding adolescent and/or adult patient (13 years and older).

Demonstrates understanding of patient care needs of the geriatric patient.

Completes all training and education to maintain clinical competence on assigned unit according to competency list for specific skills.

Demonstrates self-directed learning and participates in continuing education; attends scheduled and mandatory meetings, in services, staff meetings and completes required education.

Demonstrates awareness of self-responsibility and accountability for personal professional growth.

Abides by the Code of Conduct.

Supports the culture of service excellence throughout the organization.

Performs other related duties as assigned or requested.


Minimum Education□ Graduate of a Program in Discipline X Required

Required Skills Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.

Certifications: X Basic Life Support (BLS)

Licenses: Current license to practice as a Licensed Practical Nurse in the State of Michigan.

Minimum Work Experience Previous experience in an acute care setting preferred.
