Position Overview:
At Bloom Healthcare, our hospice nurses are the backbone of our mission to provide compassionate and individualized hospice care at end of life. Ensuring our patients' complete well-being, from their physical to their emotional and spiritual needs, is our primary focus. As a Weeknight RN, you will not only deliver paramount hospice nursing care but also educate patients and families, and become a beacon of strength during the challenging moments. Our care delivery is set in the patient's community or their place of residence.

Key Responsibilities:

  • Patient-Centered Care: Provide specialized hospice nursing care tailored to the individual needs of our patients.
  • Relationship Building: Establish and maintain therapeutic relationships with patients and families.
  • Assessments: Conduct comprehensive nursing assessments to determine patient needs.
  • Care Planning: Design and implement effective care plans based on patient needs and assessments.
  • Symptom Management: Proactively manage and alleviate pain and other symptoms.
  • Safety Protocols: Always prioritize and ensure patient safety.
  • Transition Management: Seamlessly facilitate transitions between different care environments.
  • Education: Empower patients, families, and caregivers with essential knowledge about the disease process.
  • End-of-Life Support: Offer unwavering support during end-of-life processes and address any ethical concerns.
  • Documentation: Ensure timely and accurate documentation of patient care.
  • Resource Management: Efficiently utilize available resources, ensuring optimal patient care and organizational efficiency.
  • Professional Standards: Uphold and exemplify Bloom Healthcare's standards of professional conduct and continuous service enhancement.

Basic Qualifications:

  • Graduate from an accredited nursing school.
  • Possession of a valid CO RN license.
  • Minimum of one year's experience in medical, surgical, or critical care nursing.
  • Demonstrated capability in clinical assessment.
  • Superior communication and interpersonal skills.
  • Ability to tackle problems effectively and resourcefully.
  • Detail-focused with a proven track record in time management.
  • Proficiency in computer applications and systems.

Desirable Qualifications:

  • Prior experience in hospice care or oncology nursing.
  • Deep understanding of the death and dying process, along with bereavement procedures.
  • CHPN certification or willingness to pursue.

The salary for this position is competitive and commensurate with experience. The pay range for this role in the state of Colorado typically falls between $90,000 - $105,000 annually with the potential for performance-based bonuses and other benefits. Actual compensation may vary based on factors such as qualifications, experience, and location within the state.
