JOB SUMMARY:

Your Wellness Sources is looking for a qualified Psychiatric Registered Nurse with experience
in counseling individuals with mental health illness (anxiety, depression, PTSD, etc.), dual
diagnosis, and addiction treatment. We have positions available for full-time, part-time or
Flexible. PMHNP students preferred. Your Wellness Sources provides Integrated Wellness
Services that include primary care, nutritional counseling, women's wellness exams, behavioral
health services, addiction management, or substance abuse treatment to culturally diverse
populations. The Psychiatric Registered Nurse will be part of our Behavioral Health Program. It will work in collaboration with our multidisciplinary team of providers, who are one of the main
sources of referrals for our Behavioral Health Program. The Behavioral Health Program provides
services to clients in the moderate to severe range of functional impairment, including
individual and group therapies, medication management, and education. We focus on patient-
tailored recovery and work to help our clients live their best lives.

Primary Duties and Responsibilities:
  • Provide mental health assessments, counseling, and emotional and medical status of patients; documents and reports by YWS policy.
  • Provide outpatient mental health services to promote individualized patient recovery
  • Develop individualized plans and conduct reviews as needed throughout the treatment
  • Completes all documentation relevant to activities. Complies with and ensures adherence to all documentation standards.
  • Complies with all data collection, entry, and reporting requirements.
  • Consistently demonstrates effective documentation practices and develops strategies to ensure complete patient care documentation.
  • Demonstrates effective therapy and teaching skills in leading therapeutic and education groups
  • Develops and implements health teaching for clients, families, and caregivers.
  • Effectively uses and adapts a variety of teaching resources in meeting the learning needs of patients/families.
  • Participates as a member of the treatment team to develop and implement multi-disciplinary treatment plans, monitor treatment progress and plan for discharge.
  • Assists others in developing, revising, and coordinating patient's plan of care.
  • Collaborate with interdisciplinary teams as well as external care providers or representatives from external assistance programs for continuity of care.
  • Participates in meetings and on committees and represents the Behavioral Health department and organization in community outreach efforts.
  • Participates in multi-disciplinary quality and service improvement teams.
  • Use professional judgment to ascertain and apply appropriate intervention techniques with patients in crisis.
  • Assists the client in understanding the nature of emotional disturbances and accepting the need for treatment.
  • Able to make professional evaluations, decisions, and recommendations for treatment planning and implementation
  • Supervises emergency psychiatric nursing interventions as needed. Consistently demonstrates effective leadership behavior.
  • Supports the social worker or case manager in developing a comprehensive discharge plan. Models patient/family advocacy.

Minimum Qualifications:
  • Bachelors degree required. Masters Degree preferred with two (2) or more years of experience in counseling or mental health services
  • Has the ability to develop interpersonal therapeutic relationships with psychiatric
  • patients.
  • Able to work independently and as a positive member of a team
  • Able to establish and maintain effective working relationships with mentally or emotionally disabled clients and their families
  • Excellent written and verbal communication skills
  • Communicate effectively with a variety of individuals and function calmly in situations which require a high degree of sensitivity, tact, and diplomacy.
  • Comfortable treating both routine and serious mental health illness
  • CPR certification
  • Experience working with EHR and Strong computer literacy skills
  • Ability to follow oral and written instructions accurately and be detail-oriented.
  • Ability to set priorities and complete documentation within deadlines
  • Ability to work with clients from diverse cultural and social class backgrounds
  • Knowledge of medical and psychiatric terminology
Preferred Qualifications:
  • Experience and certification in Trauma therapy.
  • Knowledge of Medical billing requirements and medical necessity re: Specialty MH
  • Knowledge of opioid replacement treatment
  • Experience and comfort in working with the homeless, substance-using, medically complex,and severely mentally ill adults
  • Knowledge of City resources re: Crisis, Dual Diagnosis, or MH services
