At Pyramid Healthcare at Mountaineer we will be specializing in Substance Use Disorder treatment. We have been in operations since 1999 and currently operate over 80 programs in 8 different states, offering a full continuum of high-quality behavioral health care for individuals suffering from substance use disorders, mental health concerns, and other behavioral health issues. Summary: Case Management and coordination of care for clients/families in residential treatment including individual case management/family case management services to support client and family engagement in treatment, group facilitation for education and skill development in collaboration with the clinical team, and admission, aftercare/ discharge planning, and referral and external provider coordination and support.

Essential Duties and Responsibilities:
• Coordination client engagement through appointments, aftercare plans, schedules and meetings in collaboration with treatment team.
• Coordination and communication with referral sources and outside providers through consistent, professional communication, assessment of needs, and documentation of contact at admission and for the duration of treatment. This may include communication or coordination for UR, legal, and employment needs for the client.
• Support of admissions process by verifying all consents are signed and admissions processes are completed for each admission, scanning all required documents into chart including hospital paperwork and medical records, and making initial contacts for referral, external provider, and family when appropriate ROIs are in place.
• Conducts client and family orientation with primary therapist
• Provides case management services for clients and families through coordinating transportation needs, providing individual and family case management sessions, assisting in development of aftercare plan, providing referral and aftercare provider contact, addressing outside provider needs, facilitating groups to support clinical services provided, and supporting clients on a weekly basis to support engagement in the program.
• Ensuring each client has an aftercare plan developed in collaboration with the treatment team and business development team that is documented in eCR in accordance with program policy. Provides aftercare plan to appropriate outside providers and contacts.
• Participates in treatment team collaboration and maintains professional, collaborative communication with counselors and treatment team.
• Documentation of client, family, referral, and outside provider contacts in Carelogic in accordance with program timelines
• Participate in mandatory corporate training, program/ clinical training, and professional development
• Other duties as assigned

Required Qualifications
Education, Licensure, & Experience
Bachelor’s degree in a human services field with a minimum of 2 years of post degree, direct care experience with the
population services or bachelor’s degree in a non-human services field with a minimum of 4 years of post degree, direct care
experience with the population served.
