REGISTERED NURSE (RN) - PATIENT-CENTERED MEDICAL HOME (PCMH) with Med-Surg experience for FORT BELVOIR renamed ALEXANDER T. AUGUSTA MILITARY MEDICAL CENTER

Full-time; M-F; 730am-430pm

NO weekends, NO Federal Holidays

The Patient-Centered Medical Home model, being implemented Armywide, is a new approach designed to improve access and continuity of care. The concept creates medical homes, or clinics, that act as a small family practice. Beneficiaries at a medical home get their own core team made up of their primary care provider -- a physician, physician's assistant or nurse practitioner, licensed practical nurses, and registered nurses. The idea is that patients will get a group of caregivers who know them well and a personalized care plan unique to them.

QUALIFICATIONS OF THE REGISTERED NURSE (RN) - PATIENT-CENTERED MEDICAL HOME (PCMH):

  • Bachelor s Degree of Nursing from a college or university accredited by the National League for Nursing Accrediting Commission (NLNAC), or The Commission on Collegiate Nursing Education (CCNE).

  • Experience: RNs must have a minimum of one (1) year of medical/surgical nursing experience, as an RN, within the last three years.

  • Demonstrate competence in starting/performing intravenous (IV) access and phlebotomy/blood draw for specimen collection or present certificate of attendance and or certificate of completing IV and phlebotomy class within 3 months of employment.

  • Current BLS from the American Heart Association or American Red Cross

  • Current, unrestricted license from any of the 50 states and U.S. Territories

  • Must be a U.S. Citizen (for access to Government computer systems)

DUTIES OF THE REGISTERED NURSE (RN) - PATIENT-CENTERED MEDICAL HOME (PCMH):

  • Performs a full range of nursing duties to include functioning independently as charge nurse in the absence of the Head Nurse.

  • Obtains initial health date in relation to the patient s psychosocial and physiological needs through observing and interviewing techniques.

  • Performs on sight and telephone triage for patients with a wide range of medical problems using established protocols.174

  • Shall have sufficient experience to be able to perform tasks relative to the neonatal, pediatric, adolescent, adult and geriatric population to include the following:Skin, head, eyes, ears, nose, throat, neck, cardiovascular system, pulmonary system, gastrointestinal system, musculoskeletal system, genitourinary system, reproduction system, lymphopoietic system, central nervous system, and endocrine system.

  • Provides educational services to patients and their families for the promotion of health promotion, disease prevention and/or detection of disease.

  • Assist in identifying patient needs and implements plan of cure.

  • Performs in accordance with prevailing standards of care. Perform appropriate assessing, planning, implementing, evaluating, reporting and documenting nursing care for neonates to geriatric patients.

  • Diagnose/recognize life threatening conditions and take appropriate actions.

  • Serves as Clinical and Telephone Triage Nurse and in the absence of the Head Nurse assumes Charge duties. Participate in unit level in-services, patient education, and performance improvement programs. Assist in the orientation of newly assigned personnel.

  • Prepares all records and reports as may be required in support of services rendered and as required by MTF s Standard Operating Procedures, Federal Law, or Army Regulation. All written reports/documentation shall be legible and shall be considered property of the government.

  • Carries out Physician, Nurse Practitioner and Physician Assistants, written and verbal orders and documents in patient s electronic medical record.

  • Assures content and correctness of prepared report and sign document to validate its content.

  • Take part in Clinical Performance Improvement Programs as required to meet the standards of the Joint Commission.

  • Participates in monitoring quality of care according to professional standards and established procedures.

  • Attend and participate in patient care reports, patient care conferences, professional staff conferences and appropriate professional activities only to the extent that such attendance and participation is related to his/her assigned cases and /or performances of contract services as determined by the supervisor.

  • Recognizes signs and symptoms of impeding emergency situations, assesses patient care needs, and institutes emergency nursing measures to include CPR.

  • Utilizes the Composite Health Care System (CHCS), AHLTA (electronic Medical record) and the Ambulatory Data System (ADS). The government is responsible for training the contractor in the use of both systems and other ADP systems implemented in the US Army.

  • Completes additional tasks as assigned by the clinic chief nurse or his/her designated representative.
