We are seeking highly organized, proactive, compassionate individuals to join our team as an LCSW Therapist to support our members, their families, and our clinical team. 

As an LCSW Therapist, you will play a crucial role in providing mental health services within our primary care setting. As a vital member of our healthcare team, the LCSW is responsible for offering a range of services, including population-based education, early intervention, and participation in the management of chronic diseases. This role encompasses conducting psychosocial assessments to identify the strengths and challenges faced by members concerning their illness, treatment, and overall life situation.

KEY RESPONSIBILITIES:
 

• Provide comprehensive mental health care and support, including the diagnosis and treatment of mental health disorders.
• Develop tailored treatment plans based on individual patient needs and circumstances.
• Regularly meet with patients for counseling and treatment, adjusting plans as needed.
• Collaborate within an interdisciplinary team, participating in treatment team meetings to discuss clinical cases and update patient treatment plans.
• Engage in continuous monitoring and coordination of behavioral health services, linking with other treatment providers both within and outside the primary care setting as permitted.
• Facilitate patient and family education and self-management skills, ensuring full participation in their treatment.
• Maintain detailed treatment and progress reports, documenting in patient medical charts.
• Other duties may be assigned.

EXPECTATIONS:
 

• Ability to work effectively in a fast-paced primary care environment.
• Excellent interpersonal skills for a multidisciplinary team collaboration.
• Proficient in brief diagnostic assessments for mental health and co-occurring disorders.
• Strong organizational skills and proficiency in Microsoft Office Suite and electronic medical records.
• Successful completion of background check and drug screen.
• Maintain compliance with all Gloria Gates CARE standards, including Conduct, Compliance, and all other policies and procedures. 

EDUCATION AND EXPERIENCE:
 

• Pennsylvania Licensed Clinical Social Worker (LCSW)
• Master’s degree in Psychology, Social Work, Counseling, or a related clinical discipline from an accredited college or university
• Knowledge of HIPAA compliance and ability to handle confidential information.
• At least 3 years of experience in the field as an LCSW.
