Position Summary:

Under the direction of the Community Health Worker Manager and the Senior Community Health Worker, the Community Health Worker will conduct culturally appropriate health education, care management, care coordination, and outreach in community-based settings such as patient homes, schools, clinics, and community centers. The Community Health Worker will also assist patients to access the variety of social and medical services they need to maintain their health. The Community Health Worker will collaborate with their clinical team to promote health and wellness and contribute to the goals and objectives of a patient’s care plan. Community Clinic, Inc. is a federally qualified health center, and we encourage applicants who are interested in promoting patient-centered care in our communities.

Key Functions & Responsibilities

  • Establish trusting relationships with patients and their families and provide general support and encouragement.
  • Provide ongoing follow-up, basic motivational interviewing, and goal setting with patients/families.
  • Conduct intake interviews with patients, including enrolling patients into, and/or referring them to, various health, assistance, and social service programs.
  • Follow-up with patients, by phone, home visits, and visits to other settings where patients can be found.
  • Assist patients with completing applications and registration forms.
  • Conduct eligibility determination, enrollment, and follow-up with uninsured patients.
  • Help patients set personal goals and attend appointments.
  • Help patients connect with transportation resources and give appointment reminders in special circumstances. Transporting patients is strictly prohibited.
  • Exhibit excellent working relationships with patients, visitors, and staff, effectively communicating CCI’s mission.
  • Work closely with medical providers to help ensure that patients have comprehensive and coordinated care. Follow-up with the patient should be continuous from initial identification through closure.
  • Be knowledgeable about community resources appropriate to the needs of patients/families.
  • Conduct the CHW tasks assigned on the Care Management Team including patient education.
  • Responsible for providing consistent communication to the Care Management Coordinator to evaluate patient/family status, ensuring that provided information, and reports clearly describe progress.
  • After training conduct the diabetes education groups.
  • Act as a patient advocate and liaison between the patient/family and community service agencies (i.e. schools, Department of Human Services, Health Care for Homeless, hospitals, support groups, etc.). 
  • Record patient care management information in the EMR (training provided) and other software no later than 24 hours after patient contact.            
  • Attend regular staff meetings, training, and other meetings as requested.
  • Manage an assigned caseload of patients.
  • Working on-site is an essential duty for the job.
  • This position may require travel between CCI sites to provide additional coverage when needed.
  • Other duties as assigned.

Minimum Qualifications

  • High School Diploma or its equivalent.
  • MUST HAVE Community Health Worker Certification
  • Current CPR Certification.
  • Ability to speak and write English fluently.  Bilingual (English/Spanish) preferred.
  • Experience working in a multi-cultural setting.
  • Experience working in a community-based setting for at least 1 to 2 years preferred.
  • Knowledge of some medical terminology preferred.
  • Computer skills required, knowledge/experience with Electronic Medical Records (EMR) is a plus.
  • Ability to initiate and maintain positive working relationships with CCI staff and other organizations.
  • Understand the community served - community connectedness.
  • Good communication skills, such as listening well, and using language appropriately.
  • Ability and willingness to provide emotional support, encouragement, and motivation to patients.
