United Homecare Services, a local non-profit, non-medical, licensed in-home care agency seeks a care coordinator assistant for our Beaverton, Oregon office. This position requires a minimum of 1 year of management experience in healthcare or social services, preferably working with the elder population. Applicant must pass state-level background check.

Applicant must have strong communications skills, strong caregiving skills, and be highly organized. Compassion for elders and family caregivers is also a must. Assistant will work at the discretion of the care coordinator for the Beaverton office. The successful candidate will have the proven ability to function as a member of a dynamic work team. Position offers ability to work flextime/part-time. 20-hour workweek Monday-Friday 10am to 2pm.

 

Assistant responsibilities include: 

Must be able to work both independently and as part of a team
Maintain professional communications via email, text, phone and face-to-face
Answer phones, and provide requested information in a timely manner
Receive client inquiries, provide in-home consultations
Maintain client and caregiver schedules in ClearCare database
Assign caregivers to clients, ensuring an appropriate match between caregiver’s skills and client’s needs
Introduce caregivers to clients
Facilitate ongoing caregiver training as needed
Evaluate client service plans and update as needed
Be available by phone 24/7 while on-call
Be on-call at least one weekend monthly
Ability to perform caregiver duties in emergency situations with short notice
Travel the 60 mile service area to perform duties (additional mileage paid)
Maintain agency records including electronic data, client files and employee files
Ensure forms are utilized and available when needed, including care notes.
Perform other office duties as needed
