Referral RN - Hospice Home Care w/ Call - Part-time

Job Description:
Responsible to assess, plan, implement, evaluate, supervise and coordinate the home care and hospice care (if applicable at location) of patients in accordance with professional standards of nursing and policies and procedures of Essentia Health. Provides direct patient care to patients requiring care for medical, diagnostic and emergency conditions.
Education Qualifications:

    

Preferred Qualifications for Hospice Referral RN:

• Experience in nursing, with emphasis on community health education/experience
• Experience supervising caregivers and/or staff
• Knowledge of Medicare Home Health & Hospice regulations
• Experience with computer databases, work processing, and spreadsheet programs
• Strong communication and interpersonal skills
• Capable of making independent decisions with the use of good critical thinking and problem-solving skills
• Ability to work autonomously and as a team
• Experience in health information management, business, or related field

The approximate base compensation range is posted, but the actual offer will reflect our total compensation and benefits package.
