Overall Objectives of Position
The Behavioral Health Integration Specialist is a core member of a collaborative care team that will provide a wide range of services to eligible Primary Care patients (individuals, couples, families, and groups) to help them overcome a range of psychological and emotional issues. They carry the clinical responsibility as the official liaison between Primary Care and applicable PCI Behavioral Health programs as well as outside behavioral health programs for the Indian population served. This job description is not an all-inclusive list of the duties and responsibilities of this position and are expected to perform all duties and responsibilities necessary to meet the goals and objectives of applicable programs.
The Behavioral Health Integration Specialist is expected to live the Purpose and Values of the Poarch Band of Creek Indians and will go beyond the call of duty to protect the health and welfare of the Tribal community it serves.

Primary Responsibilities of the Behavioral Health Integration Specialist
  • Provides assessments, crisis management, and trauma informed care services to Primary Care patients and their families on an individual or group basis, in coordination with the medical provider.
  • Provides direct screenings for common mental health and substance abuse disorders. Facilitate patient engagement and follow up care.
  • Provides coordinated care with the patient’s medical provider and when appropriate other mental health providers and actively participates in planning, organizing, and accessing existing departmental services and addresses any barriers to care.
  • Interprets patients resources and case management needs, treatment record and coordinates between providers, nurses and behavioral health to determine interventions to address in gap in care.
  • Assists medical and nursing staff as needed with coordination of care and disposition of patients from Primary Care to patients requiring placement or transfer to mental health/ substance abuse facilities.
  • Provides patient education about common mental health and substance abuse disorders and the available treatment options.
  • Systematically tracks treatment response and monitor patients for change in clinical symptoms and treatment side effects or complications.
  • Facilitates treatment plan changes for patients who are not improving as expected in consultation with the medical provider and the psychiatric consultant and who may need more intensive or specialized mental health care.
  • Routinely provides patient education regarding the connections between mental and emotional issues and the possible physical manifestations.
  • Supports psychotropic medication management as prescribed by medical providers focusing on treatment adherence, monitoring side effects and effectiveness of treatment.
  • Completes all Documentation (notes, billing, calls, etc.) in Electronic Health Record in a timely manner.
  • Attends conferences and meetings relating to up to date practices in Behavioral Health.
  • Performs other duties as assigned by the appropriate person.
Day-to-day Responsibilities
  • Maintains and promotes positive and constructive communications and activities within Primary Care, nursing services, other departments, tribal, and community agencies for purposes of coordinating and understanding of program goals.
  • Attends meetings, planning sessions, committees, and serves on teams to promote quality and performance improvement.
  • Works toward goals and future objectives of Behavioral Health (GPRA).
  • Maintains large caseloads and assesses mental health emergencies as they arise.
  • Documents electronic assessment and treatment utilizing Electronic Health Record and other pertinent software.
Education/License/Certification and Experience Requirements
  • Bachelor’s Degree in Sociology, Psychology, Social Work, or related field required.
  • Minimum of five (5) years of employment experience in a counseling role within a behavioral health setting required.
  • Must maintain current professional licensure in Basic Life Support (BLS) or obtain licensure within ninety (90) days of official date of hire in this position.
  • Must possess certification of “Mandatory Reporter Training” or obtain certification during the first week of employment.
Skills Required
  • Absolute confidentiality, adhering to HIPAA guidelines and the Privacy Act.
  • Must have a working knowledge of computers and the ability to work in a Windows environment with various software programs such as Microsoft Office software and etc.
  • Excellent time and resource management skills
  • Ability to work in a high-performance, fast-paced, high-pressure environment.
  • Must be able to work in a team environment and independently.
  • Must possess character that earns the confidence of the patients and community and minimizes public criticism and complaints.
  • Must be people oriented and relate well to people from diverse backgrounds.
Additional Requirements
  • Ability to work odd and irregular hours, as needed.
  • Must successfully pass the required criminal and character background check.
  • Ability to travel and participate in required training, leadership development, and other events.
  • Ability to adequately and successfully perform all duties and responsibilities of this position.
