Job Description
The Care Solutions Manager will be responsible to respond promptly to incoming requests for potential
services. This position will provide in-person assessments to potential clients. The Client
Solutions Manager will discuss and obtain client agreements prior to initiating services. This position
will have a warm hand-off to the Client Success Manager who will assign the case for ongoing service.
This position should be focused on achieving a high conversion rate with the appropriate case mix for
TheKey. This position will follow up on the assessment and service outcome with a quality assurance
focus. This position will escalate potentially complex cases for quality service.

Essential Duties and Responsibilities:
● Receive leads for services through FRAX, Intake Specialist, existing referral partners or our
sales team
● Contact the Client and/or family in a timely manner to evaluate the client as a match for our
services, this includes review of scope requested, and proposed hours
● Conduct in-person or virtual client assessments that include but are not limited to a view of
home safety assessments, the client’s daily routines, the client’s preferences and needs. The
assessment includes the informal and formal support systems and caregiving needs
● Set initial pricing strategy for each client based on preliminary assessment
● Ensure that assessments contain appropriate information for Client Success Managers, Staffing
Managers and, Caregiver Success Manager to understand and communicate as need to create
ideal caregiver match
● Educating potential clients and family members on TheKey’s services and which type of
services would be appropriate following the assessment, understanding the language of the
client agreement, building rapport to create engagement and obtaining agreement
● Suggest/guide potential clients to realistic schedules for optimized outcomes
● Prior to the start of care, the Care Solutions Manager ensures all agreements are signed and all
information is gathered in the client assessment and transferred to all software systems
● Determine appropriate billing rate for each client
● Marketing and client development, including making presentations to referral sources such as
local organizations, fiduciaries, Trust Officers, physician/health care groups, etc.
● Ensure all systems are updated with relevant information and signed client agreements
● Strive for high conversion rate of appropriately matched clients. Provides feedback on barriers
to achieving our ideal client
● Other duties as assigned

Required Skills, Education and Certifications:
● Bachelor’s Degree in gerontology, health care, social work or related profession from an
accredited university preferred
● Three to 5 years’ experience in health care, elder care, social work or related industry preferred
● Previous experience in an intake/assessment position
● Computer proficiency and ability to document timely and accurately notes in system related to
client visits
● Excellent written and verbal communication skills
● Driver’s license in good standing with proof of insurance
Physical Requirements:
● Ability to lift and carry up to 15-20 pounds
● Ability to sit, stand and walk for prolonged period of time throughout the work day
● Ability to use standard office equipment such as personal computer, laptop, telephone,
photocopy machine, etc.
