Pharmacist

POSITION PURPOSE

As part of the provision of pharmaceutical care, the Pharmacist provides prescribed medications, drugs, and other pharmaceuticals consistent with ethical, legal, regulatory and professional (Pharmacy Practice) requirements. Reporting to the Director, the Pharmacist is responsible for the functions of Pharmacy Practice including patient profile and MAR Management, consultation and/or verification with prescribing physicians, distributive functions, accurate dispensing documentation, and supervision of the Pharmacy Technician functions.


Requirements:

POSITION QUALIFICATIONS

Education:

Graduate of an accredited school of nursing. Bachelor's Degree is preferred.

Bachelor's degree in Pharmacy or Pharmacy D is required.

Experience:

Prior critical care or intensive care experience is preferred.

One (1) year of clinical, hospital or health system based pharmacy experience is preferred.

Licenses/Certificates:

Current Mississippi RN licensure is required.

BLS & ACLS certification is required. PALS certification is preferred.

Licensure as a Pharmacist in the state of Mississippi or eligibility for licensure via examination or reciprocity is required.


Working Conditions:

Regularly exposed to the risk of bloodborne/airborne disease; contact with patients under a wide variety of circumstances; exposed to unpleasant elements (accidents, injuries, chemicals and illness); subject to varying and unpredictable situations; able to handle emergency, crisis or pressured situations; subject to multiple interruptions; occasionally subject to extended or irregular hours; requires judgment/actions that impact patient outcomes.

Must be able to see with corrective eye wear

Must be able to hear clearly with assistance

May be exposed to infectious and contagious diseases. May be in contact with patients in a wide variety of circumstances. Able to handle emergency or crisis situations. Occasionally subject to irregular hours. May be required to wear protective equipment as necessary.


General Duties

  • Prescription verification and validation including review of patient profile, MAR, physician order, conformation with clinical initiatives; consult with physician as appropriate.
  • Distributive functions to include dispensing of medications for individual patients, dispensing and supervision/verification of technicians stocking ADC, pre-mix and post-mix admixture, and other dispensed medication storage locations.
  • Compounding functions; controlled substance dispensing, reordering and inventory accountability; management and participation in the controlled substance waste program;
  • Maintains validated competency skills as required for a staff pharmacist of the department; trains staff on Pharmacy related updates; orients, instructs, and trains assigned personnel and/or students.
  • Maintains current licensure and on Mississippi Board of Pharmacy Standards and Practices; enhances professional growth and development through participation in education programs, current literature, in-services, and meetings.
  • Monitors data and participates in the department's quality assurance and improvement plan; assures compliance with department, regulatory, and facility policies, procedures and regulations.
  • Ensures accurate, timely communication among caregivers (e.g. handoff/report, SBAR, timely communication to charge nurse/supervisor of issues) to include appropriate chain of command when indicated.
  • Timely reporting and documenting of unexpected incidents or adverse events, including participation in investigations and completion of incident reports as applicable.
  • Demonstrates knowledge and care standards for all patient populations.
  • Participates in department special projects; provides administrative support to the department director/manager; capable of department leadership in the absence of the Director/Manager.
