This position will work within the office handling intake responsibilities by phone for new clients in combination with working as a Home Health, LPN as needed traveling to client homes to assist the team as needed.

Monday-Friday scheduling - No Weekends!

Job Summary

Responsible for the processing of new resident/client requests and tracking admission status. Coordinates admissions process, completes paperwork and orients residents/clients to location.

The Licensed Practical Nurse (LPN) participate in the implementation and evaluation of patient/resident care for stable and predictable patients/residents of all ages in the home care setting. Primary job function will be clinical intake

Acts as a support person to facility by completing administrative tasks such as answering phones, taking messages and answering basic location questions. Ensures smooth transition from referring facility to location. Visits with discharge planners, family members and members of the interdisciplinary team to promote the location and the use of services provided. Enters data in to the electronic medical record (EMR) accurately.

When working in home health, will assist to develop the individual client plan of care for each client. Will be responsible for the admissions and coordination of services for clients. Coordinates admission/intake process and paperwork with client and location employees in compliance with state and federal regulations.

Ability to deliver high quality customer service in a professional manner. Strong interpersonal and listening skills. Ability to handle confidential and sensitive information. Must be able to coordinate several different responsibilities at once.

Secondary Job function would be providing home visits as back up for staff on vacation or staff calling in.

Demonstrates knowledge of nursing principles for the patient/resident in the relevant setting and possesses the ability to perform testing to assess data reflective of the patient's/resident's status and to interpret appropriately the information needed to identify each patient's/resident's requirements relative to the age-specific needs, and to provide the care needed. Demonstrates competency and practices within the full scope of nursing expertise/knowledge and utilizes appropriate age and population specific standards as designated in the home care setting. Ensures the health, comfort and safety of patients/residents.

Knowledge of the principles and skills needed for nursing to provide patient/resident care and treatment. Knowledge of medications and their effects on patients/residents. Knowledge of patient/resident education principles to develop a self-care program. Knowledge of common safety hazards and precautions to establish a safe environment, specific to the home setting.

Skill in administering a variety of treatments and medications as directed. Skill in documenting and maintaining records. Skill in establishing and maintaining effective working relationships with patients/residents, health care providers, and the public. Ability to recognize problems and recommend solutions. Ability to interpret, adapt, and follow guidelines and procedures. Ability to communicate clearly. Ability to maintain confidentiality of sensitive information. Counsels patients/residents and family members, under the direction of a registered nurse, on prevention and treatment plans.

Additional job duties such that fall under community outreach

Plans and implements strategies to identify potential sales opportunities, while focusing on increasing or maintaining clients and referral partners. Generates and follows through on all sales leads. Works on building and maintaining qualified leads through phone calls, community visits, presentations, emails and other approved tactics. Adheres to sales quality standards to build trust, uncover needs, and become a valued partner. Establishes and maintains relationships with referral sources and community leaders to promote the home based care.

Qualifications

Graduate from an accredited practical nursing program as a Licensed Practical Nurse.

If position requires IV therapy/management skills, the following is required in North Dakota: graduation from a Board approved AASPN program where IV therapy is part of the curriculum or completion of an IV Therapy course approved by the North Dakota Board of Nursing.

One year of professional nursing experience preferred.

Currently holds an unencumbered Licensed Practical Nurse (LPN) license with State Nursing board and/or possess multi-state licensure privileges. Obtains and subsequently maintains required department specific competencies and certifications. Depending on location, a valid drivers license may be required.
