Description:

Responsible for providing high-quality, direct patient care in the Countryside Care and Rehabilitation department. Responsible for pleasant and effective communication with the patients, with coworkers, supervisors, and the public.


Duties and Responsibilities

  • Ability to follow established standards of nursing practices and implement facility policies and procedures.
  • Ability to conduct patient rounds daily, reporting problems to nursing supervisor and initiating corrective action.
  • Coordinates and supervises patient care as necessary.
  • Under the direction of the Director of Nursing, monitors and makes changes in staffing according to the staffing matrix.
  • Provides or delegates care consistent with plan of care, guidelines of care and professional licensure provisions.
  • Promotes nursing professionalism, coaches and develops others.
  • Ability to identify patients’ problems and emergency situations and initiate immediate "life saving" measures in the absence of a physician.
  • Ability to follow, supervise and evaluate the implementation of a Patient's Rights Program.
  • Ability to attend staff meeting, in-service classes and committee meetings as assigned.
  • Ability to complete report for facility committees as assigned.
  • Ability to perform and document comprehensive assessment of each patient.
  • Ability to follow facility discharge/transfer procedure and coordinate discharge/transfer with other departments, physicians, and ancillary service providers.
  • Ability to assess patient needs and initiate development of individualized care plans.
  • Ability to review and revise patient care plans as necessary and as required by regulations.
  • Ability to participate in patient care conferences as assigned.
  • Ability to count all narcotics, sign for count and exchange keys.
  • Ability to prepare and administer medications, tube feedings and treatments as ordered.
  • Ability to notify the Director of Nursing, when necessary, of changes in patients' conditions, weight changes, medication errors, medication not available, abnormal lab and x-ray results and any other information as necessary.
  • Ability to assume accountability for data contained in the Personnel Policy Manual.
  • Actively engage in quality improvement activities.
  • Ability to perform other related duties as directed by the Nurse Manager.
  • Ability to assist Wound care nurse as needed.
  • Ability to verify orders.
  • Complete admission and timely manner.
  • Perform other duties as assigned.
Requirements:

Minimum Requirements

  • Must possess a high school diploma or GED.
  • Registered Nurse (RN) degree from a recognized college or university with current or pending licensure.
  • Minimum 2 years’ experience in Nursing.

Preferred Requirements

  • Bilingual (Spanish and English)
  • Experience working in Geriatrics
  • Current Advanced Cardiac Life Support (ACLS) certification
  • BSN from an accredited educational institution.

Skills and Competencies

  • Strong written and verbal skills.
  • Self-disciplined and self-motivated.
  • Effectively present information in one-on-one and small group situations to customers, clients, and other employees of the organization.
  • Understanding of medical concepts while performing nursing duties.
  • Must possess the ability to deal tactfully and effectively with patients (and their families), coworkers, hospital staff and physicians.

Physical Requirements

Perceive the nature of sounds by the ear, express or exchange ideas by means of spoken word, perceive characteristics of objects through the eyes, extend arms and hands in any direction, seize, hold, grasp, turn, or otherwise work with hands, pick, pinch, or otherwise work with fingers, perceive such attributes of objects or materials as size, shape, temperature, or texture; and stoop, kneel crouch, and crawl. Must be able to lift 25 pounds maximum with frequent lifting, carrying pushing, and pulling of objects weighing up to 10 pounds. Continuous walking and standing. Must be able to identify, match, and distinguish colors.
