Job Description of Case Manager:

Case manages and coordinate programing for adults with developmental disabilities complying with DDS, Medicaid, and DOL standards in a clinical day treatment setting.

Duties and Responsibilities of Case Manager:

• Case manages, coordinating services for new admissions including assessments, goal/objectives, prescription tracking, staffing/progress reports/data/admission/discharge
• Assess referrals, determine eligibility, obtain documents to support
• Maintain consumer files per DDS/Medicaid standards, along with mandatory reporting
• Conduct, plan, schedule Career Counseling per WIOA guidelines
• Adhering to DOL guidelines Sec, 14c
• Ability to comprehend/apply DHS, DDS, Medicaid, DOL, AND WIOA standards
• Assist with behavior issues needs/crisis intervention • Apply positive behavior practices
• Communicating with families/caregivers/PASSES/Agencies • Assist in coordinating/plan/carry out events including getting volunteers/speakers/activities
• Work closely with other departments as necessary
• Conduct tours • Assist with break duty/classes as needed
• Required annual training as directed by DDS, AUI and other regulatory agencies
• Strong attendance, team work, flexibility a must • Practice HIPAA/privacy practices a must
• Review weekly client pay run for accuracy
• Know community resources or be able to gain this information
• HIPAA privacy and handles sensitive and confidential information
• To ensure the safety of program participants, employees must be mentally alert and physically able to quickly respond to the needs of the participants
• Other duties as assigned

Minimum Qualifications of Case Manager:

• Bachelor’s degree preferred in Human Services, Social Work, Counseling, or relevant field
• Experience: 1-3 years’ experience working persons with disabilities preferred

Physical Demands -
Ability to work a regular work schedule to be available when clients are present to meet the essential functions of the job

A clinical Day Treatment Program for adults with developmental disabilities.  A work environment characteristic described here are representative of those an employee encounters while performing the essential functions of this job.
