Job Overview:

As a Referral Coordinator at Lin Health, you'll be instrumental in advancing our pain recovery program, serving as the primary clinical liaison to train and support clinic providers, staff, and patients. This dynamic role demands a committed professional with a robust clinical background, exceptional verbal and written communication skills, and the capacity to cultivate meaningful relationships with healthcare providers across all tiers.

Responsibilities:

1. Clinical Relationship Cultivation:

- Cultivate and fortify robust clinical relationships, fostering trust with all clinic members (MD, PA, NP, MA, office staff, etc.).

- Boost appropriate patient referrals from partner clinics via education, identification, effective communication, and collaboration.

2. Clinical Training & Continuous Support:

- Deliver comprehensive clinical training to referring providers and staff tailored to their respective audience.

- Extend ongoing on-site support to clinicians and staff, promptly addressing queries and concerns.

3. Collaborative Referral Drive with Partners:

- Identify and facilitate clinically suitable referrals.

- Collaborate with clinic providers and staff to ensure a seamless transition for patients entering the pain recovery program.

- Coordinate with clinicians and staff to standardize and optimize workflows and documentation specific to the Lin Pain Recovery Program.

4. Clinical Coordination with Partners:

- Ensure timely and efficient management of clinical escalations and care coordination.

- Showcase patient success stories to reinforce continuous program engagement, drive clinical practice transformation, and strengthen partner relationships.

5. Patient Engagement and Education:

- Introduce and educate patients about the cognitive pain reprogramming approach.

- Assist patients with enrollment into the Lin Pain Recovery Program.

- Provide support to patients who are currently enrolled in the program.

6. Internal Collaboration:

- Collaborate closely with Lin Health's multi-disciplinary team across various departments.

- Participate in remote and occasional in-person meetings, providing insights to enhance company tools and systems.

Qualifications:

- Clinical background (e.g., nursing, physical therapy, behavioral health, allied health) with relevant experience.

- Experience with pain reprocessing approaches is advantageous but not mandatory.

- Exceptional interpersonal and relationship-building skills.

- Willingness to travel, including for launches, follow-ups, and onsite support.

- Must be based in Durango, Colorado.
