Reporting Relationship:

Reports to the Wellness Center/Garlands HomeCare Solutions Manager.

Principle Accountabilities for our Wellness Center Nurse:

Follows established standards of nursing practice and implements facility/department policies and procedures.
Insists to interpret policies and procedures to nursing assistants, other related departments, physicians, client/Member and family members.
Obtains report from nurse being relieved and records sufficient information to implement appropriate follow up actions as necessary.
Conducts rounds if needed during shift to identify client/Member needs or care issues and initiates corrective action as necessary.
Must have all personal time off approved by GHCS Manager.
Must coordinate vacation coverage and personal time off with partner/nurse staff of Wellness Center.
Will act as the emergency on-call RN for GHCS skilled nursing cases.
Will document any case when notified on an emergency basis to Agency Manager of GHCS.
Will alternate with GHCS Manager and partner Wellness Nurse with the on-call 24/7, after hours emergency policy and procedure of every third weekend, holidays, and vacation coverage.
Communicates pertinent information to other Wellness Nurse(s) as needed.
Will schedule and maintain existing staff to current client/Member home for GHCS.
Will schedule and maintain any new Team Member to client/Member home for any new care being requested.
Will create assessment, binder for home of client/Member and instruct Team Member deployed into home, of plan of care created under GHCS licensure.
Assist training of caregiving staff as requested.
Assist with file maintenance i.e. care notes and updates.
Assist with Team Member interviews as needed.
Assist with regulatory requirements as needed.
Attend care plan meetings with family members and make recommendations.
Visit homecare Members for 90 day visits and document files.
Act as on-call nurse for licensure.
Communicate well with GHCS Manager and or family members of any changes of Member with documentation.

COMMUNICATION

Prepares wellness information for Garlands Gazette by the 12th of each month.
Communicates all activities forthcoming to the Activity Department.

SERVICES TO MEMBERS

Update Access system with Member’s medical and other information after interview.
Conduct wellness assessments for prospective IL and occasionally Prairieview prior to move-in.
Meet with new Members within 3 months of move-in to set up file with current Member information and advise Members of activity, fitness, and wellness programming along with information on GHCS.
Maintain Members medical record file, update and keep secure.
Visits hospitalized IL Members or those who recently returned home as an ambassador and medical concierge.
Help Members navigate through their medical issues and communicate with Members’ physicians and pharmacy as needed to clarify issues for the Members.
Give Member referrals for MD’s, Hospice, Home Care, Home Health, etc.
Coordinate and schedule on-site physician service i.e. Audiologist, Dentist, Optometrist, and Podiatrist.
Schedule laboratory testing. Complete paperwork in preparation for the blood draw or other testing.
Teach Members how to correctly use medical devices, blood sugar machines, inhalers, incentive spirometry, nasal sprays, nebulizers, etc. as needed.
Initiate contact with all Members who return from hospital or skilled nursing stay within 2 days of return to community.
Fill medication boxes for Members receiving medication concierge services.
Obtain MD orders and update internal file with medication lists on a regular basis for those receiving medication concierge services.
Order prescription refills for Members if needed.
Collect outdated medications from Members and hand off to Member Services for disposal.

NURSING SERVICES

Coordinate annual flu clinics for Members.
Set up and administer flu shots via flu clinics to Team Members.
Administer TB Skin test for all new GOB Team Members and annual testing as needed.
Check vitals for Members upon request within the center or home.
Respond to all emergencies during hours nurse is present in the building i.e. falls, illnesses, first aid, injuries. Services are to include Members and Team Members.
Perform wound care/dressing changes or special procedures (i.e. tube drainage) for Members within center or home.
Administer medication for Members who request assistance i.e. eye/ear drops.
Remove sutures and or replace bandages from Members upon request.
Evaluate wounds and make referrals i.e. home health, physician, and emergency room.
Arrange blood draw on Tuesday mornings and obtain MD via email/fax.
Consult with family members regarding any medical issues or concerns as needed.
Monitors technology and assures daily response to any unusual occurrences.

WELLNESS EDUCATION

Arrange and/or conduct two yearly wellness lectures for Members in IL.
Schedule and reserve space for speakers on Member wellness events.
Assist in coordination of Member wellness events.
Research and perform sample health foods program monthly, coordinate selection with Executive Chef.
Assist with yearly health fair and other educational programs.
Provide assistance/advice related to communicable disease issues community-wide as they arise, to include documentation.
Perform internet research for Members who request written information related to health and wellness issues.
Works with Human Resources to develop and implement wellness programs for Team Members.

ADMINISTRATIVE

Prepare Incident and Concern Reports and broadcast to Incident Contact List.
Maintain medical supplies inventory.
Submits Wellness Nurse billing for billable nursing services to the accounting department.
Obtain and maintain records on a yearly basis, external physician licensures and insurance forms.
Meet outside vendors, Hospice, Medicare, Home Health, medical supply companies, etc. as requested.
Attends weekly Transition Meetings when on duty.

Knowledge, Skills, & Abilities for Wellness Center Nurse:

Experience working in assisted living, home care or long-term care environment preferred.
Licensed in the state of Illinois as a Registered Nurse.
Must possess, or be able to obtain current CPR certification.
Must be able to speak and write English.
Must be able to work independently, as well as a team member, with a positive, pro-active attitude.
Is flexible and adapts to change and new tasks.
Follows all policies and procedures of The Garlands.
Attends meetings and transition meetings.
Must be flexible, highly customer service and people oriented individual who demonstrates compassion, patience, and understanding, and is willing to do whatever it takes to provide the highest level of service.
Must be a team-oriented individual who enjoys working collaboratively with others, creating a positive and uplifting environment, and communicating very effectively at all levels.
Must be able to respond quickly and appropriately to an emergency situation.

Physical Requirements:

Must be able to manually lift and carry up to 50 lbs.
Must be able to assist in transferring residents who weigh in excess of 100 lbs. occasionally.
Must be able to tolerate long periods of sitting, walking, bending, and standing.
Must be able to push Members in wheel chairs for brief periods of time.
