* Collaborate with other professionals, such as social workers and healthcare providers, to ensure holistic care for clients
* Monitor clients' progress and make necessary adjustments to their care plans
* Advocate for clients' rights and ensure they receive appropriate services
* Maintain accurate and up-to-date documentation of client interactions and progress
Experience:
* Bachelor's degree in social work or a related field (Master's degree preferred)
* Previous experience in case management or a related field
* Knowledge of motivational interviewing techniques
* Familiarity with community resources and services
* Strong communication and interpersonal skills
* Ability to work effectively in a team environment
* Excellent organizational and time management skills
