Description:

We are seeking a passionate and dedicated Community Health Worker (Managed Care) to join our team. The Community Health Worker will play a vital role in connecting individuals and families in our community with the resources and support they need to achieve optimal health outcomes. This position involves collaborating with healthcare professionals, community organizations, and local residents to address health disparities, promote preventive care, and enhance overall wellness.

Requirements:

General job duties

  • Conduct outreach and engage with community members to assess their health needs and connect them with appropriate resources and services.
  • Provide health education on topics such as chronic disease management, nutrition, mental health, and preventive care through individual and group sessions.
  • Advocate for the needs of community members and collaborate with healthcare providers, social service agencies, and other stakeholders to address health disparities.
  • Assist individuals in navigating the healthcare system, including scheduling appointments, obtaining insurance coverage, and accessing medications.
  • Facilitate support groups and community events to promote health promotion and disease prevention.
  • Maintain accurate records of client interactions, referrals, and outcomes to track progress and evaluate program effectiveness.
  • Participate in ongoing training and professional development opportunities to enhance knowledge and skills in community health promotion.

Qualifications


  • High School Diploma or its equivalent.
  • Experience working with culturally and economically diverse populations.
  • Successful completion of a Community Health Worker formal training program such as from a college or other education institution within one year of date of hire. Maintain this certification.
  • Successful completion of a Mental Health First Aid Training, preferred.
  • Ability to be insured on the commercial auto insurance policy based on driving record including active driver's license and personal automobile insurance and reliable transportation.
  • Employment offers conditional upon completion of a background screen including fingerprinting and a drug screen
  • Within the first 90 days of employment: BLS and completion of health screenings as outlined in the credentialing policy are required for successful completion of orientation.
