RN Case Manager

Job Summary:
We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our healthcare team. As a Registered Nurse Case Manager, you will be responsible for providing comprehensive patient care and coordinating services for individuals in need. This position requires strong clinical skills, excellent communication abilities, and a commitment to delivering high-quality patient service.

Duties:
- Perform patient assessments to identify healthcare needs and develop individualized care plans
- Coordinate and manage patient care across various healthcare settings
- Collaborate with interdisciplinary teams to ensure continuity of care
- Monitor patient progress and make necessary adjustments to care plans
- Educate patients and their families on disease management, treatment options, and available resources
- Conduct diagnostic evaluations and interpret test results
- Administer medications and treatments as prescribed by physicians
- Maintain accurate and up-to-date patient records in compliance with HIPAA regulations
- Monitor vital signs and observe changes in patient condition
- Provide primary care services to patients of all ages, including pediatrics

Requirements:
- Valid Registered Nurse (RN) license in the state of [LOCATION]
- Bachelor's degree in Nursing (BSN) preferred

Salary: $85.00 - $90.00 per hour
