Job Summary

Under the direction of the Program Director of the assigned County in collaboration with other members of the treatment team, the Case Manager will arrange for all necessary medical, educational, habilitative, and rehabilitative and social services for the client and family. The Case Manager position will perform secretarial duties that will include operating a multiline telephone system, scheduling appointments, and compiling and recording medical charts, reports, and correspondence. This position requires the ability to work independently, exercise judgement, and take an initiative.

Education/Experience:

  • Require an Associate's degree with 3 years' experience working under CASSP principles, or;
  • Bachelors / Master's degree in the Social Sciences or another related field of study.
  • One-year experience in case management or counseling with children and families, preferred.

Job Duties / Responsibilities

  • Schedule and attend all evaluations and ISPT meetings
  • Attend all meetings, trainings, hearings, weekly supervisions, etc.
  • Develop and send out ISPT invites and letters to all necessary individuals
  • Complete any and all necessary paperwork, packets, reports, documents, client tracking forms, etc. required by employer, MCO or MA for each individual client in a timely manner
  • Inform parents and all treatment team members of any changes or updates to cases
  • Prepare client information for evaluators prior to the scheduled client evaluation
  • Assist in case assignments for BC/MT staff and scheduling of BHT
  • Track treatment plans and discharge due dates
  • Distribute and collect BHT update forms prior to the client's evaluation
  • Audit IBHS charts regularly and review for accuracy and completeness of all charts
  • Substitute for assigned BHT when necessary
  • EVS / insurance checks for all IBHS clients
  • Communicate with insurance companies regarding client's benefits and authorizations to ensure proper coverage of services
  • Performs a variety of clerical and administrative duties including, but not limited to; ordering and maintaining inventory supplies, typing, proofreading, filing, faxing, scanning, data entry, copying, and recording information in accordance with deadlines and company procedures
  • Schedules and confirms patient appointments, insurance information, and medical consultations. Communicates canceled appointments accordingly and makes all necessary attempts to fill open appointments immediately
