The VNA & Hospice of the Southwest Region is looking for a full time Hospice Clinical Supervisor for Rutland and Bennington County.

A DAY IN THE LIFE OF A HOSPICE CLINICAL SUPERVISOR

This position is responsible for providing day to day supervision of the clinical staff in VNAHSR's Hospice Program. Responsibilities include but are not limited to:

Key Responsibilities:

  • Collaborates and assists the Hospice Clinical manager with daily responsibilities such as checking daily referrals, assigning cases to staff, and ensuring client visits are made.
  • Provides clinical oversight to perform hospice plan of care reviews to ensure quality care and optimization of SIA reimbursement.
  • Coordinates and oversees new hospice employee orientations including, but not limited to the completion of all required competencies at 90 days of service, with a detailed development/performance plan for the first year of service.
  • Fosters professional, respectful relationships through coaching, supervision, active listening, giving and receiving feedback, etc.
  • Performs on-site reviews of nursing staff on an on-going basis (every six months, or as needed based on performance).
  • Performs hospice QI reviews on admission, recertification and discharge, ensuring compliance with processes.
  • In collaboration with Hospice Clinical Director, schedules, plans, coordinates, and delivers monthly team meetings and debriefings. Takes action to ensure that meetings provide staff members with a variety of educational, clinical and operational information in an engaging and open environment.
  • Serves as a Hospice RN as needed to meet operational needs. Expected to stay current on the essential functions of the RN position at all times.

QUALIFICATIONS:

  • Graduate of accredited school of nursing. BSN preferred.
  • Five years of clinical experience with a minimum of two years in a supervisor position required. Prior experience in Hospice highly desired.
  • Valid Vermont State Nursing license required. Valid state driver's license required. Ability to successfully pass pre-employment driver record, criminal, adult and child abuse background check/review required. Must show proof of auto insurance at time of employment.
