Now Hiring: Community Health Worker - Maternal and Child Health

(Grant-funded position)


The Community Health Worker (CHW) will work closely with individuals and their infant(s) in the community during pregnancy, the postpartum period, and/or up to the child’s third birthday. The CHW will work in tandem with a Public Health Nurse to provide personalized, community-based care to these clients. The CHW will assist clients in finding community resources which reduce their barriers to care and help ensure that the client is knowledgeable about their health and the health of their infant(s). This position is grant funded through a Community Partnership Grant. 


Duties Include:

  • Client outreach and enrollment activities with pregnant and postpartum individuals.
  • Conducts home and community visits with families and infants from diverse cultural, socioeconomic, educational, racial, and ethnic backgrounds.
  • Identifies needs based on the social determinants of health and provides appropriate resources, referrals and assistance.
  • Educates clients about the importance of preventative health care; provides basic education to ensure an understanding of their health status and conditions.
  • Accurately documents all client contact in an Electronic Health Record System including using mobile technology for partial remote work and on home visits.
  • Attends local events and disseminates health related information throughout the community.
  • Other related duties as assigned including participating in emergency preparedness or response activities.


Requirements:·        

  

  • High school diploma or GED is required. Bachelor’s degree in public health, public administration, social work or other human services related field preferred.
  • Current and unrestricted Community Health Worker certification through the Ohio Board of Nursing required.
  • OBGYN, prenatal and/or postpartum preferred.
  • Knowledge and proficiency using an Electronic Health Record system preferred.
  • Must possess a valid Ohio driver’s license and be insurable through FCPH’s provider; some evening and weekend hours may be required.


 

Hiring Salary: $20.00/hour to $24.06/hour. This is a non-exempt position.
