Job Description:

Sunbelt Staffing is seeking an experienced Registered Nurse (RN) for a travel contract assignment in home health care in Hammond, LA. This opportunity allows you to explore a new city while utilizing your nursing skills to provide high-quality care to patients in their homes.

Responsibilities:

• Deliver skilled nursing care to patients in accordance with physician orders and agency policies.
• Conduct comprehensive assessments, develop individualized care plans, and implement appropriate interventions.
• Administer medications, treatments, and procedures as prescribed, ensuring accurate documentation of all care provided.
• Complete OASIS documentation within a 24-hour turnaround time and maintain accurate patient records using Homecare Homebase.
• Collaborate with interdisciplinary team members to coordinate patient care and promote positive outcomes.
• Educate patients and families on disease management, medication administration, and home safety practices.
• Uphold professional standards of nursing practice and protect patient confidentiality at all times.

Requirements:

• Active Registered Nurse (RN) license in Louisiana or compact state.
• Minimum of 1-2 years of home health experience as an RN.
• Proficiency in Homecare Homebase charting system.
• Ability to achieve 25-30 productivity units per week, with compensation for a full 40 hours.
• Flexibility to work Monday-Friday with potential for weekend shifts or on-call duties.
• Reliable transportation, valid driver's license, and active car insurance.
• Compliance with Joint Commission standards, including background check, drug screen, and medical documentation.
